If you’re ready to enroll your child at The Little Learning Preschool, we’re excited to welcome you to our family! Get started by submitting your initial registration with the button below. Once your form is submitted and processed, you will receive any applicable enrollment fee invoices withing 24-48 hours. If you have questions or concerns, contact our team now.

INITIAL REGISTRATION

ADMISSION FORMS

Once you have completed the Initial Registration form, you can start the admissions paperwork. Only start these forms after the Initial Registration form is completed. Read the description of each form and then click the link to complete it. Get in touch if you have questions at any time!

REQUIRED DOCUMENTS FOR ALL FAMILIES

Admission Form (Required)

Complete a separate admission form for each child you are enrolling.

Emergency Card (Requried)

Complete one form for each child you are enrolling.

Photo Release Form (Required)

Complete one per family.

Parents Rights (Required)

Please review and e-sign.

CACFP (Center ID 1519) (Required)

Required food program form. To complete this form, our center ID is 1519. Our name is The Little Learning Preschool and our phone number is (817) 862-7555.

2025 Parent Handbook

Download and save our 2025 Parent Handbook for your records. Please read before signing the agreement form.

2025 Handbook Agreement (Required)

Initial and sign this form after reading the Parent Handbook. Only one form is necessary per family.

ONLY COMPLETE IF APPLICABLE FOR YOUR CHILD

**Food Allergy Form

This form is REQUIRED for any child with a diagnosed food allergy. Download it for the child’s physician to fill out.

**Infant Safe Sleep

For infants ages 6 weeks - 11 months old ONLY.

Health Statement

This form is required for children who do not currently attend PreK or school away from the center. Send a copy of this form for your child’s physician to complete. You will have 30 days from the date of enrollment to submit.
